I don’t have one on hand, but try looking at project proposal templates. They are what I have used to create mine in the past. I don’t have a generic one is because each one is client / project specific. Here are high level things to include, scope, goals, deliverables, time frame, cost. Leave a place for both dated client signature as well as yours.
